Provides human services which assist individuals and families to achieve self-sufficiency and social well-being. Services are provided directly by department personnel and through collaborations or contracts with other community agencies. The Department takes leadership in community efforts tp assess human service needs, plan for more effective and efficient human services delivery, and address emerging community needs.
Adolescent Response Team (ART)-Lar. Cty. Dept. of Human Services
An intake unit that primarily works with families of youth aged 11 to 18 who are in crisis. ART provides mediation and immediate crisis assistance to address family conflict, then follows up with six sessions within 30 days and assessment for out-of-home placement. Referrals are generated either.
Adolescent Services, Community Service - LCDHS
A program that diverts youth from the juvenile justice system and prevents them from being incarcerated in adult facilities. May offer juvenile offenders the opportunity to provide useful public service to the community as an alternative form or restitution for breaking the law.
Adolescent Services, Court Ordered-LCDHS
Court order Adolescent Services provides case management to youth ages 13 to 18. The youth referred to this program are involved with the court system and are either parties to a Dependency and Neglect petition or are adjudicated delinquents. Services provided include out-of-home placements, day treatment, and emancipation training.
Adolescent Services, Ongoing - Lar. Cty. Dept. of Human Services
Provides help and information about emancipation services to youth, and information and qualification about the Chafee Foster Care Independence Program. There is no formal procedure necessary to become emancipated. To be considered emancipated, the juvenile must be at least 16 yrs of age and living indepent in matters of care, custody, and earnings, or be married or in the military.
Adult Eligibility-Larimer County Department of Human Services
Assists needy and disabled adults in accessing various financial assistance programs such as: Aid to the Needy Disabled (AND); Colorado Supplement (C/S); Old Age Pension(OAP); Qualified Medicare Beneficiary (QMB); Special Low-Income Beneficiaries (SLIMB); and Medical Assistance for Long-Term Care.
